EMPLOYEE ELIGIBILITY:

All full-time Park Pro employees are eligible for Medical, Dental and Vision benefits on the first of the month following 60 days of employment.

DEPENDENT ELIGIBILITY:

Dependent children are eligible through the end of the month following their 26th birthday.

HOW AND WHEN CAN YOU MAKE CHANGES TO YOUR ENROLLMENT STATUS?

Open enrollment will occur once a year. Each year you may change your coverage level.  Once you have made your selection, you may not change it until the next year's open enrollment unless you have a "life changing event." Life changing events include:

  • Change in marital status
  • Change in the number of dependents
  • Change in employment status
  • Dependent's satisfying or ceasing to satisfy dependent eligibility requirements
  • Significant Cost Changes
  • Significant Curtailment of Coverage
  • Addition or Improvement of Benefit Package Option
  • Change in Coverage of Spouse or Dependent under another Employer Plan
  • Loss of other health coverage
  • Judgment, decree, or order
  • Entitlement to Medicare or Medicaid

In the event that you have a "life changing event", you must complete the necessary paperwork and notify the Administrator within 30 days if you wish to change the status of your insurance coverage.
